That contrary to the terms of the agreement, Defendant failed to remit payment as promised. 10. That the last payment received by Plaintiff on the subject loan was on or before June 30, 2022, for the June 30, 2022 payment. 11. That as a result of Defendant material breach of contract, as of September 23, 2022, Plaintiff was owed the principal amount of $3,101.09. 12. That Plaintiff has previously demanded Defendant remit payment as promised and Defendant has failed and/or refused to do so. (Exhibit B) 13. That the contract provides Defendant shall be responsible for Plaintiff's reasonable attorney fees incurred in having to bring this action. Waiver Public Notice Michigan Department of Health and Human Services Behavioral and Physical Health and Aging Services Administration Flint Waiver Amendment Request The Michigan Department of Health and Human Services (MDHHS) plans to submit a waiver amendment request to the Centers for Medicare & Medicaid Services (CMS) for approval. This request will allow MDHHS to continue coverage of targeted case management services for all eligible pregnant women up to twelve months of post-delivery who are: Served by the Flint water system or were served by the Flint water system between April 2014 and the date on which the Flint water system is deemed safe by the appropriate authorities; AND Pregnant women with household income above 400 percent federal poverty level can buy into unsubsidized coverage under the program. The anticipated effective date for the Flint Waiver Amendment is April 1, 2023.
